Annual TB Requirements:

To keep your TB certification current, please download the below forms, complete them, and submit them to your branch office prior to your TB certification expiration.

TB Requirements for Lapsed Certification

If your TB certification expires prior to your completing and submitting the above, you will be required to take additional steps to bring your TB certification current. Staff with expired TB certification are suspended and unable to work during suspension. To bring an expired certification current, the below are required:

New TB skin test, blood test, or chest x-ray (to be completed by a licensed medical provider)
TB Education
Risk Assessment
Symptom Screening (questionnaire)
