To expedite the training process, we suggest that you compile the below list of items while waiting for your background approval:
- CPR card (if applicable). Please don’t worry if you are not yet CPR certified; we will train you.
- Visit your local health department, doctor’s office, or walk-in clinic to obtain a TB skin test and bring results to training.
- Obtain direct deposit verification or voided check from your bank.
- When you come to training, bring 2 forms of government identification (i.e. driver’s license, passport, social security card). When submitting your driver’s license, please be sure it is valid, including name and address updates.
- If working for Forte Residential, you will also need to provide proof of current auto insurance (must contain your name). If you will be working for Forte Home Health Care only, or if you will not be driving while working for Forte Residential, a waiver will be provided for you to sign, in place of proof of auto insurance.
- Certificate of completed HCSP training required by Indiana Medicaid. Before training at the Forte office, HCSP training must be completed online. You will receive information to create an account once you are approved for hire. Information about the training can be found at https://www.in.gov/fssa/ddars/files/HCSP-FAQs-Created-6.30.25.pdf Please note: This does not apply if you are applying to work home health services only or Attendant Care or HCA services. Please check with your branch office if you are unsure whether this applies to you.
